A Youthful Transformation – Part 1

As we age, changes in bone structure and soft tissue—such as sagging skin—can naturally lead to an older, more tired appearance. For those seeking noticeable rejuvenation, surgical methods remain the most effective solution.

Our patient is a 44-year-old woman who wished to achieve a more youthful and refreshed appearance. Despite her age, she appeared older due to several key factors: drooping eyelids, a long and downward-sloping nose, and overall facial sagging, particularly along the jawline.

She also expressed a strong desire to achieve a slimmer facial contour by narrowing the width of her cheekbones and reducing the mandibular angle (the area just below the ears). Taking her goals into account, Dr. Jeong designed a comprehensive surgical plan that combined facial contouring with a face-lift and neck lift to address both structure and skin laxity.

Given the complexity and number of procedures involved, it wasn’t advisable to perform all surgeries at once. Therefore, Dr. Jeong approached the transformation in stages. On the first day, she had eye surgeries that could be safely performed under sedation anesthesia—specifically, upper eyelid revision and lower eyelid correction.

The patient was concerned about droopy upper eyelids and puffiness under the eyes, which made her look tired. She also wished for a more defined double eyelid line, requesting an “out-line” style that is higher and more visible.

|🩺 Surgical Plan
Dr. Jeong performed upper eyelid revision to lift the droopy lids and refine the double eyelid line into an out-line shape. This was combined with canthoplasty to enhance the overall eye shape.
For the lower eyelids, sagging and under-eye hollowness was addressed through fat repositioning. Additionally, Dr. Jeong performed OOM (orbicularis oculi muscle) suspension, which secures and tightens the muscle to create a firmer, smoother appearance.

This photo was taken on Day 11 post-operation, during her final check-up before returning abroad. The improvements around her eyes were already visible—even at this early stage of healing.

To achieve a brighter, more youthful gaze, Dr. Jeong corrected both the drooping skin and the inner corner of the eyes through refined canthoplasty.
For the lower eyelids, the tear trough ligament was released to smooth out the hollow, sunken areas. The bulging fat was redistributed into this space, and excess skin was removed. Lower eyelid muscle was also lifted and fixed, resulting in a taut, rejuvenated under-eye contour.
